CHCS Duals Demonstrations Webinar Materials Available

On October 30, the Center for Health Care Strategies hosted a webinar on state progress with the duals demonstration. The webinar provided on-the-ground perspectives from three of the first states to launch demonstrations. Speakers from Minnesota, Virginia and Washington State – representing alternative, capitated and managed fee-for-service models – shared successes, challenges, lessons learned and thoughts about what lies ahead.

New Research Finds Well-Targeted Care Coordination Yields Return on Investment

A new report by Avelere for The SCAN Foundation makes the case for three best practice strategies on effective care management of high-risk Medicare beneficiaries:

  1. Identify the full range of risk factors: non-medical factors are just as important as medical, so health plans must focus on risk profiles using a variety of data sources beyond traditional claims or financial data
  2. Improve data collection with a well-developed health risk assessment: health plans should invest in a comprehensive health risk assessment process which can help improve risk stratification and care coordination
  3. Implement targeted care coordination programs to drive outcomes and ROI: research revealed the potential for substantial cost savings when transitions are managed throughout the continuum of care
