A Person-Centered Diabetes Prevention Model

This edition's column is from Rosa Palacios, Consumer Engagement Specialist with the Center for Consumer Engagement in Health Innovation at Community Catalyst. Prior to joining the Center, she was the Consumer Involvement Coordinator and Stanford Chronic Disease Self-Management Specialist at Commonwealth Care Alliance (CCA). One of Rosa’s responsibilities was to implement and to bring to scale the Chronic Disease Self-Management Program at CCA. She has a Master Trainer Certificate in the Diabetes Self-Management Program in both English and Spanish issued by Stanford University.
